In order for us to determine if the ASC has authority to look into this matter and can provide any assistance, please complete the following Initial Complaint Assessment form. All information provided will be treated as confidential and used only as permitted under the Securities Act (Alberta).

If you know for certain your complaint is ASC related, or if you've been instructed to do so, you can create a more detailed complaint – this provides a bit more information so that we can better assess and respond to your complaint.
